You can register via the internet; please properly fill in the following form.
Once you have sent the form, please forward us a cheque at this address:

The amount of the cheque to be sent is indicated on
the descriptive page of the each program.
In some cases, a second cheque must be delivered as per the date indicated.
Your place will be guaranteed once your cheque/cash has been received.

*FOR 3 ON 3 HOCKEY WE ARE ACCEPTING A DEPOSIT CHEQUE OF $100 TO RESERVE YOUR INDIVIDUAL SPOT OR A CHEQUE OF $250 TO RESERVE A 10 PLAYER TEAM SPOT IN OUR LEAGUE. ALL 3 ON 3 MONEY MUST BE SUBMITTED BY APRIL 31st TO SECURE A SPOT AFTER THIS DATE YOU COULD BE PLACED ON A RESERVE LIST BASED ON SPACE.

IF YOU ARE SUBMITTING A TEAM PLEASE PRINT OUT THE FORM BELOW WITH YOUR 12 PLAYERS AND 1 GOALIE AND SUBMIT YOUR CHEQUE WITH THE FORM. **PLEASE NOTE ALL OF YOUR TEAM WILL STILL NEED TO REGISTER THROUGH THE INDIVIDUAL LINK BELOW AND DETAIL THEY ARE ON YOUR TEAM. REMEMBER NO REGISTRATIONS ARE COMPLETE UNTIL WE HAVE RECEIVED YOUR PAYMENT IN FULL.

Each player, parent and/or guardian frees the BluePrint Hockey. organizers, instructors and/or volunteers of any and all responsibility of accidents, injury or loss suffered by the participant. Therefore, the player, parent and/or guardian agrees to release and discharge any and all claims, actions and proceedings which may be brought against BluePrint Hockey organizers, instructors and/or volunteers for any accidents, injury or loss arising out of the player’s participation.

I hereby authorize the directors or instructors of BluePrint Hockey to make a decision in regards to the emergency care required for my child.
